STEP 4: COMPLETE THE REQUEST FOR CE CREDIT FORM
STATE INSURANCE CONTINUING EDUCATION CREDIT

This information applies ONLY to those wishing to earn STATE INSURANCE CE CREDIT.

Request for Continuing Education Credits must be sent to the College the same week you pass your exam.

Because state insurance CE credit is managed by individual states, there are variances in requirements and procedures. The College for Financial Planning has developed a 4-Step procedure to help you receive the state insurance CE credit you earn.

STEP 2: CHOOSE A MONITOR/PROCTOR

Exam Monitor/Proctor Requirements and Forms

Prometric is approved in all states as a disinterested third party test facility.

A monitor or a proctor is an individual who serves witness that you have satisfied the requirements of your course and testing procedures in an ethical manner. The proctor must be someone with whom the examinee has no business or family relationship. Business relationship is further defined as working within the same company regardless of physical location, title or position.

A proctor cannot be a currently enrolled student in any College for Financial Planning courses.

States requiring that your exam monitor/proctor be pre-approved by the state's Department of Insurance:

  • New York and South Carolina - Prometric is approved as a proctor. You may also contact your state's department of insurance for a list of approved proctors, or submit your proctor using the form for New York or South Carolina.

Identify the Appropriate Monitor/Proctor Affidavit of Personal Responsibility
Once you have an individual appropriately identified and approved by your state (if required), you must also submit an Affidavit of Personal Responsibility.

If you choose to test at Prometric and will be requesting state CE please be sure to review the Prometric affidavit and use in place of the affidavit related to your state.

States requiring specific Affidavits of Personal Responsibility include:

For all other states, use the College's Affidavit of Personal Responsibility unless testing with Prometric.

For ALL states, the proctor and student must sign the affidavit, immediately after submitting the exam. The signed affidavit must be submitted with your request for CE

STEP 4: COMPLETE THE REQUEST FOR CE CREDIT FORM

Download the printable "Request for Education Credit" form. Complete submission instructions are included on this form.

By submitting this request form, you are requesting that your completion of a state approved course be reported to your State Department of Insurance.

Immediately after completing the exam, the proctor and student must sign the appropriate affidavit of personal responsibility. The signed affidavit must be submitted with the most current Request for Continuing Education Credit.

Failure to meet these submission requirements will result in the rejection of CE requests.
